Aim: It has been recently reported that advanced age, obesity and diabetes mellitus have reportedly been connected with development of non alcoholic liver disease (NAFLD) which are also associated with COVID-19 infection. Poor nutritional status (PNI) is also associated with severe COVID-19 infectrion but its pathogenic link between NAFLD and coronavirus disease remain elusive.
Methods: At total, 80 patients ( mean age 56.6 years and 45 female ) with confirmed coronavirus disease between April 1 and august 30, 2020 were enrolled in a pandemic response center of a tertiary university hospital in coastal Blacksea, Turkey. Ultrasonography results of the abdomen with laboratory parameters at least two years before diagnosis were obtained and screened retrospectively. Patients were divide as ultrasonography proven NAFLD and those without. Comparisons between the NAFLD and no- NAFLD groups were performed with the Chi-Square test.
Results:There were no gender and age difference between groups ( p>0.05). In NAFLD group, the mean serum albumin level was 4.12 mg/dl and mean serum lymphocyte level was 2060 per milliliter, in non-NAFLD group the mean serum albumin level was 3.82 mg/dl and 1540 per milliliter. NAFLD was associated with better prognostic nutritional scores compared to patients with normal liver ultrasonography (56.6 versus 49.7; p<0.005) but had no effect on length of hospital stay (LOS) (8 days versus 6 days; p=0.53). The mean serum glucose level was significantly higher in NAFLD group (112 versus 139 mg/dl ;p= 0,031).
Results: These data indicate, for the first time, that better PNI was associated with NAFLD in coronavirus patients. Our data can also be used for evaluating disease severitiy in metabolic diseases during viral infections. In summary, NAFLD, common in western world, is not assoiated with increased LOS even in hyperglicemic status.
